Psychiatric Nurse Practitioner

GENERAL DESCRIPTION  
 
Under the administrative direction of the Executive Director and the clinical supervision of the Medical Director, the Nurse Practitioner works as a member of the Tenderloin Outpatient Clinic’s “Med Team” (along with 1 psychiatrist and 2 other psychiatric nurse practitioners) to provide various direct and indirect psychiatric clinical services to clients of a multidisciplinary nonprofit adult outpatient community mental health clinic. 
 
RESPONSIBILITIES  

  • Psychiatric evaluation of new clients for diagnosis and initiation, alteration or continuation of psychiatric medications, as well as risk assessment 

  • Maintain a caseload of individual adult clients for medication and symptom monitoring, including administration of long-acting injectable medications, with follow-up appointments being a combination of in-person and telehealth 

  • Refer clients as needed to on-site individual therapy, groups, case management, and neuropsychology 

  • Participation in weekly team and staff meetings and in-service trainings 

  • Supervision and consultation with medical director, staff, and interns 

  • Adequate productivity via hitting service billing targets (5400 minutes of service per month per full-time employee, pro-rated for part-time employees) 

  • Timely electronic health record documentation in Epic (notes entered within 3 days) 

  • Adherence to all state, federal and county documentation standards and HIPAA regulations 

 
QUALIFICATIONS  

  • Possession of current Nurse Practitioner license in good standing issued by California Board of Registered Nursing 

  • ANCC certification as a Psychiatric/Mental Health Nurse Practitioner (PMHNP) 

  • DEA license in good standing 

  • Strong knowledge of DSM-5 criteria, HIPAA, and psychiatric medication treatment options 

  • Computer literacy and comfort working with electronic health record; experience with Epic preferred 

  • Commitment to working in a community mental health setting with challenging clients with severe, acute and chronic psychiatric disorders, including dual & triple diagnosis

Clinical Director

Hyde Street Community Services (HSCS) is currently recruiting for a Clinical Director. This is a full-time, exempt position reporting to the Executive Director. HSCS is committed to providing a robust additional benefit package to complement compensation. This package includes:


JOB DUTIES AND RESPONSIBILITIES

  • Adheres to nationally accepted standards of practice, utilizing evidence-based guidelines.

  • Oversees outpatient behavioral health care, waitlist and social services to complex patients and grievances.

  • Attends BHS Adult Directors Meeting and disseminates/updates clinical procedures

  • Collaborates with the clinic and quality improvement leadership team on the development, implementation and maintenance of clinic quality improvement programs as well as admin binder and clinical audit.

  • Collaborates on the development, implementation and/or approval of treatment protocols.

  • Collaborates with Training and Medical Directors to provide consultation and supervision for staff and students including selecting, training, coaching, and mentoring.

  • Oversees and participates in partnership with the Medical Director and FSP Supervisor in the quality improvement process including medical chart reviews/chart audits.

  • Provides support and consultation to the Clinic Coordinator and Compliance for managing operations of the Clinic and patient concerns.

  • Assures compliance with requirements set forth by the City of SF DPH and Medi-Cal.

  • Requires broad theoretical knowledge and expertise in the practice of a variety of treatment modalities and diagnostics and therapeutic techniques in order to diagnose and solve problems in psychotherapeutic work with adults.

  • Requires knowledge of the racial/ethnic, economic, and social factors affecting the residents of SF served by the program.

  • Carries a small caseload of patients (50%).

  • Supervises staff and students as needed.

QUALIFICATIONS  

  • L.C.S.W., LMFT, PhD or Clinical Psychologist

  • License to practice in the state of California

  • Experience in upper management, oversight and providing supervision

  • Three or more years’ experience in an outpatient health and/or behavioral health clinic setting. Experience in a community health center setting working with underserved populations preferred

  • Demonstrated experience with DPH compliance and quality improvement programs

  • Experience with development and administration of programs, and staff supervision

  • Demonstrated experience with EPIC Electronic Medical Records

POSITION DESCRIPTION

HSCS Clinical Director is a full-time, exempt position overseeing the outpatient clinical services. The Clinical Director provides oversight of comprehensive outpatient services delivered to a diverse and underserved patient population with co-morbidity or dual diagnoses. The CD directs the staff and oversight of the organization's clinical programs. This position works closely with clinical staff, department directors, and all HSCS Programs to ensure that the highest standards of quality and service are maintained. The CD oversees the implementation of quality improvement efforts designed to improve clinical performance and maintain compliance which meets all credentialing standards as set by the agency. The CD evaluates and is responsible for compliance, standards if behavioral health care and social services and makes recommendations for new practices and procedures. The CD also advises the Executive Director and medical teams on matters impacting patient care.

GENERAL DESCRIPTION   
Under the administrative direction of the Executive Director and the clinical supervision of the Medical Director, the Staff Psychiatrist works as a member of the Tenderloin Outpatient Clinic’s “Med Team” to provide various direct and indirect psychiatric clinical services to clients of a multidisciplinary adult outpatient community mental health clinic.
 
RESPONSIBILITIES 

  • Psychiatric evaluation of new clients for diagnosis, risk assessment, and psychiatric medication planning

  • Maintain a caseload of individual adult (18+ yo) clients for medication and symptom monitoring, including administration of long-acting injectable medications, with follow-up appointments being a combination of in-person and telehealth (phone and video)

  • Refer clients as needed to on-site individual therapy, groups, case management, and neuropsychology
  • Participation in bimonthly team and staff meetings and in-service trainings

  • Case consultation with medical director, non-medical staff, and interns
  • Back-up supervision for Nurse Practitioners and Psychiatric nursing staff when medical director is on leave

  • Adequate productivity via hitting service billing targets (5400 minutes of direct service per month per full-time employee, pro-rated for part-time employees)
  • Timely electronic health record documentation in Epic (notes entered within 3 days)

  • Adherence to all state, federal and county documentation standards and HIPAA regulations

  
QUALIFICATIONS   

  • Possession of current DEA number and medical license in good standing issued by the Medical Board of California  

  • ABPN eligibility, with certification preferred   

  • Strong knowledge of DSM-5 criteria and psychiatric medication treatment options  

  • Computer literacy and comfort working with an electronic health record  

  • Commitment to working in a community mental health setting with severe, acute and chronic psychiatric disorders, including dual diagnosis and medical comorbidities, housing instability, and some with history of recent incarceration
